story of the misaligned incentives and apparent greed that cost the
lives of the crew of the Challenger Space Shuttle mission in 1986.
Government officials and corporate executives felt great pressure (based
on their own incentives) to launch under dangerous conditions
when key engineers like Allen McDonald refused to sign off on the launch
due to safety concerns. They were overruled and nearly silenced in an ugly coverup, were it not for McDonald's courage to speak out as a
whistle blower. The rush to launch really bother me when I think about this book. </span><div class=""><br class=""></div><div class=""><span class=" break-words
"><span dir="ltr" class="">Just as engineers at Morton-Thiokol were demanding that the Jan. 1986 launch be delayed because the conditions were not right for launch and there were of
genuine concerns about the potential failure of the system due to O-ring trouble, so also the
"engineers" of the patent system -- the IP workers who rely on that
software for many aspects of their work and apparently some internal EBC staff at the USPTO -- have been warning against the launch of the
Patent Center as the sole software platform for patent work due to many
bugs that are far easier to see than the subtle problems with O-rings on
