<p class="MsoNormal">I’ve had luck just sending a letter and Part B form that was marked up to indicate I just am paying the difference. I make sure to specify exactly how much I’m paying in the letter and add the language “please apply any unpaid fees or
unpaid balance to Attorney Docket XXX”. I don’t select any fees at the fee page.<o:p></o:p></p>
