I’d send a letter to Apple’s General Counsel. It will find its way to the right person.

A nonprofit I work with is having trouble with a scammy app. Any advice on how to find a human at Apple who can act would be greatly appreciated; it's not purely a trademark issue, as there is a prominent disclaimer, but it is monetizing our users' content in violation of our terms of service and also providing false information to the site/Cloudflare in order to bypass our blocks.
Thanks so much.
