    the e-signature service for the Combined Declaration of Use and
    Incontestability Under Section 8 and 15 for application registration
    number <mumble>, from Trademark Electronic Application System
    (TEAS), U.S. Patent and Trademark Office.

    To submit Combined Declaration of Use and Incontestability Under
    Section 8 and 15 electronically, please click Submit.

Yes one of these arrived in our office a few minutes ago for one of our 
trademark cases.
