It looks like you are using the WRONG form-- i.e., a Request to Make Special instead of a Petition to Make Special.

There is no fee if it’s a request to make special due to an inadvertently canceled registration.

If it’s a petition, then it’s $250 and you’ll probably get a phone call or email from the PTO to request payment.
