[Pct] How best to remove duplicate page from a PCT?

A PCT application was recently filed with X pages, and the receiving office somehow duplicated one of the pages. A notice was received indicating that a fee is owed for having one more page than what was paid for. If you look at what the PTO has on their end, it has one of the pages in there twice even though the uploaded version did not have that duplication.
Question: is it better to request that the duplicate page be deleted, or to file a replacement specification? Does it matter? (Asking for a friend, of course!)
Wrinkle: in addition to the above, the PCT was filed with an un-numbered cover page inadventently included (and correctly included in the page count and fee that was paid at the time). The practitioner doesn’t want that cover page to stay in the record, which is part of why there is a desire to file a replacement specification that omits the unintended cover page, as well as to hopefully get rid of the mysterious duplicate page.
Personally, I have never understood the cover-page practice – perhaps as a side note someone can enlighten me about why some practitioners use them. It was never part of my training when I was a wee young practitioner, so it always strikes me as odd.
