[Patentpractice] Modifying PTO forms - must I remove the OMB notice

You can also just leave it all unchecked and generate your own 
transmittal -- although that will cause you to lose any PTA.

My understanding is that if you modify the form, you have to remove OMB. 
HOWEVER -- if you just type extra things on top of the form, you're not 
modifying the form, you're just unconventially filling it out, like 
making a note on a paper form.
