Just stumbled across this today (actually when looking at new guidance 
re: GUI Icons on Design Patents)

I think it's pretty self-explanatory re: previous discussion re: 
certificate of transmission referring to EFS-Web or PatentCenter

USPTO accepts pre-printed forms with a certificate of transmission via 
EFS-Web when a form is transmitted to the USPTO via Patent Center
