[Patentcenter] indicating fee amount that one wishes to pay

When I am using a credit card (which is almost always how we pay our
fees), I sometimes find I have no choice but to use Form 2028 and fax it
to the Central Fax Number +1 571-273-8300. there is a wide free-text
box toward the bottom of the form where I type in the four-digit fee
codes that I am paying.
